We are so excited to welcome you as an Arts Education Fellow in the 2018-19 ACTIVATE cohort and have you join our ever-expanding network of powerful arts advocates.
TO ACCEPT THE FELLOWSHIP AND CONFIRM YOUR PARTICIPATION IN THIS YEAR’S ACTIVATE PROGRAM, PLEASE DO THE FOLLOWING:
- Take a moment to review the 2018-19 ACTIVATE Program Schedule to be sure you can attend all of the scheduled sessions. Please note that the LA Convergence (November 2, 2018) and ArtsMonth activities (April 2019) are optional.
- Download, carefully review, and sign the 2018-19 ACTIVATE Fellow Letter of Agreement, which contains important information about the expectations and requirements of the ACTIVATE Fellowship. Please note that your agreement to the Photo & Video Release section of the form is optional. However, agreement to the ACTIVATE Fellowship Expectations and Requirements is required.
- Complete the online 2018-19 ACTIVATE Arts Education Confirmation Form below before 12:00pm on MONDAY, September 10, 2018. Please note that the online confirmation form requires that you submit a brief bio and contact information for your Fellow Profile, which will be shared with the rest of the 2018-19 ACTIVATE cohort. IMPORTANT: you MUST upload a signed copy of your letter of agreement when completing the online confirmation form.
IMPORTANT:Given the limited number of spots in the ACTIVATE cohort, the selection process was very competitive. If we do not receive your confirmation by 12:00pm on MONDAY, SEPTEMBER 10, 2018, your spot will be given to the next person on the waiting list. Please note that your place in the 2018-19 ACTIVATE Cohort is not considered confirmed until you complete and submit the form below.
